Valera Health Expands Opportunities for Integration of Behavioral Health into the Primary Care Setting
New York, NY. August 1, 2016 –Brooklyn-based behavioral health startup Valera Health is expanding efforts to better integrate behavioral and physical health through a new multi-phase project that uses patient self-assessments and personalized tools to support recovery and wellness for people with depression and anxiety. The project, occurring in partnership with Montefiore, a leader in population health, is part of a broader strategy to leverage the latest technologies to improve patient supports and engagement, particularly for individuals with co-morbid physical and behavioral health conditions.
“Valera Health is proud to collaborate with Montefiore and its high-performing accountable care organization to help improve patient engagement and recovery for those with behavioral health needs and chronic illness. Augmenting the collaborative care integration model with digital health tools is exciting,” said Thomas Tsang, M.D, CEO and co-founder.
Valera’s platform utilizes a patient-facing smartphone application that is connected to a cloud-based care management dashboard. In addition to being a conduit for patient self-assessment, education and engagement materials, the platform can detect the changes in patient behavior that suggest the need for clinical intervention and improved care planning.
“We know that integrating behavioral health care into primary care is instrumental for improving patient health outcomes, and are proud to be a leader in advancing this model of care,” said Henry Chung, M.D., Vice President and Chief Medical Officer of Montefiore’s Care Management Organization.
