Valera Health Partners with Mount Auburn Cambridge Independent Practice Association (MACIPA) in Innovative Care Management
Cambridge, MA – Mount Auburn Cambridge Independent Practice Association (MACIPA) and Mount Auburn Hospital are partnering with Valera Health, a digital health startup, by using new technologies to help improve engagement for patients suffering from chronic medical conditions and depression.
This project will evaluate the effectiveness of a smartphone application in engaging patients suffering from chronic medical conditions such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and depression. The Valera Health mobile app enables health coaches to communicate with their patients via a HIPAA-secured chat messaging or video call on either iOS or Android-powered devices. The app also administers clinically-validated surveys to regularly assess patients’ symptoms and mood. Lastly, the Valera technology platform monitors relevant biometric data such as steps taken per day and weight change. Armed with these data, health coaches are better able to care for their patients by responding proactively to concerning changes as soon they are detected.
By augmenting standard care with the introduction of patient reported outcomes (PRO) and smartphone usage patterns, care providers are able to better manage chronic medical conditions. This proactive approach may lead to fewer urgent care visits, emergency room visits, and hospitalizations. David Mou, MD, MBA, Medical Director at Valera Health explains, “The goal is to make health delivery systems more preventative and less reactive.”
Nathalie Bloch, MD, assistant Medical Director at MACIPA, is heading this innovative project. She is interested in introducing technologies that can both improve care while reducing unnecessary healthcare spending. “We would like to bridge the gaps in between the office visits to a continuum of care and to increase patients’ engagement. We believe that this kind of intervention will lead to a better outcome in chronic disease management.”
